Request for Newfound Advocacy

 1.  Please provide the following information at our initial meeting. 

a. Who is involved? Give your child’s name, age, and grade.

b. What it is that you want to happen? What service(s) or program do you want to obtain?

c.  When did the concern originate or is this a new concern?

d.  Where did this occur?  What school district is involved?

e.  Why are you concerned?  Why is this important?

 2. A copy of your child’s I.E.P. and assessment information needs to be provided.

 3. A check or money order payable to Newfound with an initial payment of $140.00 for the first consultation is required.

 4. If you are outside of the Dallas Metroplex area, please check to see if this would be an area that we would travel to.
